About this role
A Social Worker in Endeavor Health’s Palliative Care and Symptom Support Program delivers psychosocial support to patients and families experiencing serious illness. The role serves patients across settings (patient residences, nursing facilities, and Endeavor Health offices) and works closely with an interdisciplinary team. The position reports to the Practice Manager of Palliative Care and supports program goals for symptom management and palliative services.

- BLS - American Heart Association (AHA)
