About this role
The Medical Social Worker at On Lok PACE within the Institute on Aging provides psychosocial assessment and therapeutic counseling to older adults and adults with disabilities who have complex medical and psychosocial needs. The role serves as the primary liaison between participants, families, and the interdisciplinary care team to coordinate participant-centered care and support aging in place. The position also facilitates enrollment in PACE, connects participants to community resources, and advocates for participant needs.
