About this role
The Pharmacy Medication Historian compiles accurate and complete patient medication histories by interviewing patients and family members and recording prescribed, over-the-counter, and supplement use. Works primarily in emergency room and inpatient settings to verify medication information with pharmacies and physician offices. Identifies medication discrepancies, facilitates corrections or escalations to pharmacists, and communicates pertinent findings to the care team.
