Mid-Level Medical Provider

The Mid-Level Medical Provider supports individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This role ensures that medical and nursing practices meet internal policies, regulatory standards, and community best practices.
