MnCHOICES Assessor

The MnCHOICES Assessor conducts person-centered assessments to determine eligibility for long-term services and support under Minnesota’s MnCHOICES Assessment Program. The role ensures compliance with state and federal guidelines and collaborates with individuals, families, and community partners to develop care plans that promote independence, health, and well-being. This position works under the limited supervision of a Social Services Supervisor to coordinate appropriate services and resources.
