Performs discharge coordination and post-discharge follow-up for mothers and newborns across the maternal-newborn continuum, promoting throughput and care coordination. Provides both direct and indirect care before and after discharge, facilitates referrals and resources, and supports compliance with state maternal health requirements and HCAHPS expectations.
