MSH Case Manager

The Case Manager is responsible for managing all aspects of care for an assigned group of patients, including determining the appropriateness of the level of care, developing and implementing care plans, and identifying expected lengths of stay. This role involves collaborating with physicians, social workers, and other interdisciplinary team members to reduce length of stay, improve efficiency, and ensure timely discharge planning. The position also requires conducting utilization reviews and coordinating with managed care companies to support patient progression through the continuum of care.
