About this role
The MVPP Engagement Specialist works within the Multi Visit Patient Program to engage and support identified multi-visit patients in the Boston Medical Center Emergency Department. The role focuses on building trust with patients affected by chronic homelessness, substance use disorders, and severe and persistent mental illness, and connects them to Population Health resources. This is a full-time overnight position reporting to the Senior Manager of Behavioral Health within the Population Health department.
