SHIELDS for Families
The Navigator POSC provides prenatal and postpartum support to women with substance misuse or substance use disorders through hospital and home visits. This role involves conducting assessments, enrolling participants, and collaborating with families and hospital staff to develop individualized Plans of Safe Care. Navigators offer education on maternal and infant health, connect families to behavioral health and community resources, and maintain accurate documentation and program data.
