About this role
The Nurse Care Manager is a nursing professional embedded in the Behavioral Health Home (BHH) Adult and Children’s Service Teams, providing clinical consultation, oversight of health outcomes, and collaboration with primary care partners. The role supports recovery-oriented, prevention-focused care to improve health outcomes for adults and children with serious and persistent mental illness. It also involves interpretation and reporting of health data and coordination within an integrated health and wellness team.
