About this role
Telephonic Nurse Case Manager II provides care management for members with complex and chronic health needs, primarily via phone, to assess, develop, implement, coordinate, monitor, and evaluate individualized care plans. The role supports member access to appropriate services, facilitates authorizations and referrals, and collaborates with medical staff to optimize care across the continuum. This position is largely virtual with required in-person training and occasional on-site duties such as hospital discharge planning.
