About this role
Assumes 24-hour accountability for managing Access, Care Coordination, and Discharge Planning functions and serves as a central resource to the Director of Case Management. The role develops and implements strategies to ensure timely, patient-centered care that supports appropriate financial reimbursement and optimizes clinical outcomes. Collaborates with disciplines, payers, system partners, vendors, and community agencies to align practice with ethical, legal, and regulatory standards.

Qualifications
- Graduate of Accredited Registered/Professional Nursing Program
