About this role
A Nurse Practitioner (Adult and/or Gerontology) providing high-quality, primary care to seniors and other residents in skilled nursing, assisted living, and independent living settings, including telehealth as needed. The role practices in a person-centered, high-touch model to diagnose, treat, and manage acute and chronic conditions while collaborating with interdisciplinary teams and families. Position is based at Allison Pointe Healthcare Center in Indianapolis, IN with a Monday–Friday daytime schedule and no weekends or on-call duties.
