About this role
The Nurse Practitioner (Advanced Practice Provider) will join Mass General Brigham Medical Group to support a new Medicaid-focused wrap-around clinic, delivering primary care to patients in community and home-based settings primarily in the South region (Pembroke). This is a full-time, 40-hour position responsible for managing a panel of approximately 400 patients and providing continuity of care across home, community, and some in-clinic visits.

Qualifications
- Master's Degree Nursing
