The Resource Nights & Weekend (ReNW) Nurse Practitioner at Chapters Health provides fully remote virtual serious-illness care management across the organization’s continuum, including home-based, facility-based, inpatient hospice, palliative care, and PACE programs. The NP is an active member of the medical staff who provides nights/evenings/weekends coverage and supports continuity of care for adult and pediatric patients and families.

Qualifications
