About this role
A Community-Based Palliative Medicine Nurse Practitioner at Montage Health provides specialty palliative care for community patients with serious or chronic illness, working within the Palliative Medicine Service and collaborating with an interdisciplinary team and supervising physicians. The role is community-facing and includes coordination of palliative services and education for patients and providers. This is a temporary, onsite position with driving required and assigned 8 10-hour shifts per pay period.
