An Advance Practice Nurse / Acute Care Nurse Practitioner position at Southlake Health within a new Extensivist Clinic delivering intensive outpatient, patient-centered care. The Clinic is an interdisciplinary program that bridges inpatient and outpatient services to prevent avoidable admissions and support earlier discharges. The role is based on the Newmarket hospital campus and is part of a team providing continuous, seven-day-a-week integrated care.
