About this role

Eternal Faith is seeking a Nurse Practitioner for Palliative Care Services to assist physicians in overseeing clinical care and treatment for patients and families. The role involves consulting with attending and hospice physicians, providing clinical direction to the Interdisciplinary Team, and completing face-to-face documentation and home visits for palliation and management of terminal illnesses. The Nurse Practitioner serves as a key member of the Interdisciplinary Group, reporting to the Hospice Administrator.
