Nurse Practitioner – Primary Care / Geriatrics

HealthPlus Staffing is seeking an experienced Nurse Practitioner for a full-time opportunity providing comprehensive primary care to older adults in Stockton, California. This position operates within a PACE model of care and focuses on providing coordinated, patient-centered care to a frail and elderly population. The Nurse Practitioner will work closely with the Medical Director and interdisciplinary care team while providing direct patient care, developing individualized care plans, and coordinating care across multiple settings.
