Nurse Practitioner – Primary Care

This role involves providing comprehensive primary care and medical assessments to senior participants, focusing on holistic treatment and care coordination within a collaborative interdisciplinary team. The Nurse Practitioner works under the direction of the Medical Director to develop care plans, manage discharge planning, and oversee effective care coordination for participants in their homes or nursing facilities. The position requires maintaining continuous medical coverage and participating in rotating night and weekend call to ensure ongoing patient support.
