Nurse Practitioner

Meritus Health is seeking a Nurse Practitioner to lead a high-autonomy house-calls program focused on wound care and longitudinal primary care for patients in their homes and at the Home Health clinic. The role involves managing a defined patient panel, coordinating with interdisciplinary teams, and providing comprehensive in-home and clinic-based care with significant day-to-day independence.
