Nursing Discharge Navigator

The Nursing Discharge Navigator coordinates and facilitates safe, efficient, and patient-centered discharges to ensure continuity of care and reduce readmissions. This role involves arranging follow-up appointments, conducting post-discharge outreach, coordinating prescriptions, and providing comprehensive discharge education. A key focus is proactively supporting timely discharge planning to reduce length of stay, particularly for patients with Heart Failure.
