Occupational Therapist (OT)

The Occupational Therapist provides in‑home occupational therapy services to patients, following physician orders and a plan of care. Working under the supervision of the Director of Clinical Services/Nursing Supervisor, the OT conducts evaluations, creates and updates treatment plans, and documents progress while coordinating with patients, families, and other healthcare staff.
