Onsite Community Specialist

The Onsite Community Specialist supports members in navigating the healthcare continuum by assessing their needs, providing education on benefits and resources, and developing care plans to improve health outcomes. This role involves collaborating with internal and external partners to remove barriers to care, connecting members with community resources, and fostering trusting relationships to enhance member retention and well-being. The position requires strong problem-solving, collaboration, and motivational interviewing skills within a Florida Blue Center environment.
