Outreach Case Manager

The Outreach Case Manager actively participates in the crisis-to-housing continuum at Oasis, providing street canvasing, assessments, safety planning, and case management to homeless and street youth in the Metro Nashville area. This role involves assisting youth in creating plans to avoid homelessness, facilitating problem-solving, and connecting them with essential community resources. The position also includes supervising youth in the Drop-In Center, documenting client interactions, and participating in after-hours on-call rotations.
