The Outreach & Enrollment Supervisor at Habitat Health leads the Sacramento Center’s outreach and enrollment function, supporting the organization’s mission to enable independent aging in place through the PACE program. This role is a mission-critical leadership position that partners with center and Growth leadership to drive new participant growth, uphold regulatory standards, and represent the team in the local community. The supervisor helps ensure a seamless experience for prospects and participants while supporting team development and cross-functional initiatives.
