Outreach Support Worker

The Outreach Support Worker provides trauma-informed, nurturing support to children and young people with moderate to extreme needs, helping them build positive relationships and develop everyday skills. This role involves conducting needs assessments, managing case planning, and collaborating with service providers to promote safety, stability, and positive life outcomes. The position is part of the Assertive Outreach Support program within the Out of Home Care team.
