Outreach Therapist

The Outreach Therapist provides comprehensive community-based clinical, case management, and case coordination services for children and adolescents aged 3 to 18 who are at risk of or have a history of hospitalization. The role focuses on stabilizing and maintaining clients in their homes and communities through strength-based assessments, treatment plans, and crisis intervention. This position involves working collaboratively with families, schools, and other service providers to improve relationships and support client well-being.
