The Outreach Worker will locate, engage, and enroll Health Home eligible individuals identified by the NYS Department of Health, conducting community-based outreach such as home visits, calls, and correspondence. The role involves documenting activities in the Health Home EMR, completing intake paperwork, and responding to patient needs in coordination with program staff.
