Outreach Worker

This position creatively and effectively engages individuals living on the street through outreach, utilizing a person-centered, harm reduction approach to assist with housing placements. The role involves serving as a case manager for homeless adults, performing intakes and assessments, and advocating for needed benefits to promote client wellness and housing retention. The position requires working in conjunction with MOC teams to meet clients where they are at using motivational interviewing techniques.
