Outreach Worker

The Outreach Worker provides community-based outreach, engagement, and care coordination to adults experiencing serious mental illness, co-occurring substance use disorders, or homelessness. Working primarily in the field, the role involves building trust, assisting with housing and benefits access, and linking clients to medical and behavioral health resources. The position supports clients in achieving stability and progressing toward individualized recovery goals using a trauma-informed, person-centered approach.
