PACE Enrollment Specialist

The PACE Enrollment Specialist supports a healthcare organization in the San Fernando Valley by educating seniors and their families about the Program of All-Inclusive Care for the Elderly (PACE). This community-facing role involves building relationships with local organizations, generating prospective participant leads, and guiding eligible individuals through the enrollment process. The specialist will conduct ongoing outreach, maintain a pipeline of prospects, and coordinate assessments and enrollment activities.
