PACE Nurse Practitioner

The PACE Nurse Practitioner assesses, initiates, provides, and coordinates direct care for participants in the Care Guide Partners PACE program, primarily seen in the clinic or day center but also in other settings like homes or hospitals. The role involves supporting and coordinating care with the interdisciplinary team and other medical providers, while participating in education, outreach, and leadership activities. The position may include additional patient care or administrative duties as assigned by the Medical Director and/or Executive Director.
