PACE Primary Care Physician

The PACE Primary Care Physician provides comprehensive, participant-centered primary and geriatric care to frail older adults enrolled in the Program of All-Inclusive Care for the Elderly. The physician manages a panel of participants, delivering longitudinal care, conducting medical assessments, and coordinating with an interdisciplinary team to ensure evidence-based, cost-effective treatment across various care settings. This role involves clinical oversight, transitions of care management, and adherence to CMS PACE requirements to support participant health and safety.
