PACE Social Worker

The PACE Social Worker provides direct social work care management services to participants in the North East Medical Services PACE program under the supervision of the PACE Center Manager. This role involves collaborating with the Interdisciplinary Team to manage long-term care needs, conducting psychosocial assessments, and facilitating care transitions. The position requires maintaining thorough case management records and providing supportive counseling to participants and their families.
