About this role

The Participant Experience Liaison connects PACE participants to WelbeHealth’s system of care and serves as a primary contact between participants, families, and the interdisciplinary care team. The role focuses on onboarding participants into the PACE program and providing ongoing support throughout their WelbeHealth journey. It supports participant satisfaction initiatives, service recovery, and the grievance process while coordinating care across providers.
