Patient Care Coordinator

This role involves maintaining a continuous presence at the patient's bedside to ensure direct oversight and immediate safety. The position requires offering empathetic reassurance and gentle de-escalation for individuals experiencing distress, agitation, or confusion. Additionally, the role entails proactively removing visible hazards and accurately recording behavioral observations to report changes in patient condition to the supervising RN.
