Patient Care Manager

The Patient Care Manager will oversee hospice patient care services, ensuring high-quality, compliant delivery across the location. Reporting to the Executive Director or Administrator, the role coordinates scheduling, triages referrals, leads interdisciplinary meetings, and mentors clinical staff. The manager also drives quality improvement initiatives and ensures adherence to federal, state, and company regulations.
