Patient Coordinator (RN)

The Patient Coordinator (RN) serves as the primary point of contact for patients in value-based care programs, providing proactive clinical support, education, and care coordination to improve outcomes and reduce hospital readmissions. This role engages patients early in their healthcare journey to ensure they understand their care plan, access necessary resources, and receive appropriate support throughout the episode of care. The coordinator builds trusted relationships with patients and caregivers to identify and address clinical or social barriers that could lead to complications.
