Patient Educator and Case Manager

This role functions as an onsite Patient Educator and Case Manager within the Home Therapies dialysis health care team, ensuring quality home patient care and supporting company quality goals. The position involves training and empowering dialysis patients on peritoneal and home hemodialysis therapies, conducting initial home visits, and providing ongoing education to patients and their families. The role requires collaboration with clinical staff, physicians, and management to coordinate patient care, manage medical records, and maintain equipment competency.
