Patient Liaison

The Patient Liaison serves as the primary point of contact for BILH Pharmacy's Home Infusion Services, stationed across system hospitals to build clinical relationships and facilitate patient transitions to home care. This role involves engaging with patients and families to introduce services, assess therapy appropriateness, and educate on medication administration while collaborating with case managers and prescribers to resolve barriers to discharge. The position ensures seamless communication between hospitals, providers, and the pharmacy team to support safe and efficient patient care.
