Patient Liaison

The Patient Liaison serves as the primary point of contact for BILH Pharmacy's Home Infusion Services, stationed across system hospitals to build clinical relationships and facilitate patient transitions to home care. This role involves engaging with patients and families to assess therapy appropriateness, educate on medication administration, and resolve barriers to discharge. The position requires collaboration with case managers, prescribers, and clinical teams to ensure seamless communication and payer approval for home infusion services.
