Patient Navigator PRN

Employees will coordinate services like scheduling, pre-certification, and follow-up calls while participating in multi-disciplinary rounding on inpatient oncology units. The position focuses on transitioning patients between inpatient and outpatient areas while connecting them to necessary social and financial support services.
