Patient Navigator

The Patient Navigator collaborates with the Integrated Health Care team to address social determinants of health and improve patient access to care and outcomes. This role involves connecting patients with community resources, coordinating benefits enrollment, and providing education and support for chronic health concerns. The position requires building trusting relationships with patients and working closely with clinical teams to eliminate barriers to care.
