Patient Navigator

The Patient Navigator provides support, guidance, and assistance to patients and families navigating complex healthcare environments, with a focus on increasing satisfaction and optimizing care outcomes. This role involves acting as a communication liaison to understand individual needs, guiding patients to relevant services, and promoting healing and satisfaction. The position is assigned to the Citywide Peer Team, offering intensive case management services to adults with severe mental illness and co-occurring disorders.
