Patient Navigator

The Patient Navigator provides support, guidance, and assistance to patients and families navigating complex healthcare environments, with a focus on increasing satisfaction and optimizing care outcomes. This role involves acting as a communication liaison to understand individual needs, guiding clients to relevant services, and promoting healing through community integration and recovery-focused support. The position is assigned to the Citywide Peer Team, serving adults with severe mental illness and co-occurring disorders.
