Patient Navigator

The Patient Navigator serves as the first point of contact for individuals seeking care, guiding patients and their families through the admissions process at the Pennsylvania Access Center. This fully remote role involves responding to referrals, gathering clinical and demographic data, scheduling assessments, and coordinating transportation and community resources. The position requires a compassionate and detail-oriented professional to support the Intake & Needs Assessment team in a fast-paced environment.
