As a Patient Outreach Specialist at DaVita, you will serve as the first point of contact for patients, managing both Care Coordinator and Engagement Specialist duties. Responsibilities

Facilitate care dispensing hearings via telehealth and in-person coordination
Perform outbound calls for patient scheduling and engagement reminders
Coordinate patient referrals and ensure access to eligible care resources
Maintain accurate and confidential documentation for the care team
Escalate new patient concerns to Nurse Practitioners as appropriate
