About this role
The Care Coach 1 evaluates the needs of members to achieve and maintain optimal wellness by guiding them towards appropriate resources. They employ various strategies to support members' wellness while monitoring patient care and resolving barriers to effective care. This position reports to the Manager of Care Management and follows established procedures and regulations.

Qualifications
- Bachelor’s degree in Health and Human Services or a related field
- Minimum of two years’ experience in pediatric and/or early childhood education
