Peer Navigator

The Peer Navigator collaborates with the contract manager and HIV clinic team to engage and re-engage clients who have lapsed in their HIV care. This role involves conducting field outreach, providing comprehensive assessment and linkage services, and supporting individuals in maintaining consistent treatment and healthy living. The position requires acting as a liaison between testing sites and medical clinics to ensure seamless enrollment and continuous support for people living with HIV.
